Program of All-inclusive Care for the Elderly (PACE) - New Mexico
InnovAge operates the PACE program to offer All-inclusive Care for the Elderly on behalf of the federal government. This program is an alternative to nursing facilities, which includes a customized daily plan to meet the healthcare and social engagement needs of older adults 55 and older. Locations in California, Colorado, Florida, New Mexico, Pennsylvania, and Virginia. This program provides: - Coordination and delivery of healthcare services - In-home services to complement services provided at local PACE centers - Transportation for healthcare - Recreational and social activities - Meals Most program participants are enrolled for little to no cost. This program is free for older adults that are eligible for Medicaid.

-This program helps adults 55+ years old. -Must be certified by the State of New Mexico to a level of care equal to nursing home services. -Must be able to live safely at home. -Must meet income eligibility requirements.
